PAYMENT TERMS *

(I AGREE) Membership is recurring and charged on the monthly anniversary day. New members will be charged on the day of joining and accrue all benefits on that day. A charge will be made again in the month following and benefits will accrue on that day. If the scheduled payment date is an invalid date for the current month, such as when the next scheduled payment date falls on the 31st of the month, and the current month only has 30 (or 28) days. In this case, the payment will be processed on the actual last day of the current month.

NOTICE TO CANCEL *

(I AGREE) True REST does not have long term contracts. Members may cancel at any time, but will be charged one final time on the recurring payment date. Upon cancellation of Membership all accrued Float Sessions will be redeemable for 6 additional months from time of cancellation, and will expire after this date; all other benefits will cease at cancellation. Should the contract be broken by a member and payment is not made on the final recurring payment date these terms will be considered breached and one final payment will be owed (subject to collections) or one float benefit on account will be deducted (if applicable). Please send cancellation requests to GLENDALE@TRUEREST.COM. All other forms of cancellation requests are not valid. (I AGREE) UNLIMITED MEMBERSHIPS Members who purchase the Unlimited Membership or Weekday Unlimited Membership may float for up to 1 hrs/day. Unlimited Membership holders may float during any available day or time on the schedule. Weekday Unlimited Membership holders may float at any available time Monday through Thursday, at or before 4pm. For all Unlimited membership levels, memberships may not be shared. Memberships will only be honored at the spa in which they were purchased. Members on either Unlimited membership plan may purchase additional floats outside of these restrictions for themselves or a guest at the $59 member price. (I AGREE) LIMITED MEMBERSHIPS Members who purchase the Limited membership receive 1 60-minute float per month. Floats do not roll over and expire monthly. Limited Membership holders may float during any available day or time on the schedule. Limited membership level, may not be shared. Limited Memberships are location specific and will only be honored at the spa in which they were purchased. Limited Membership holders do not receive any additional membership benefits. Limited memberships are a promotional membership and as such, they are subject to a 90-day membership period prior to cancelling

Membership Cancellation & Final Billing *

(I AGREE)Membership cancellations take effect at the end of your current billing cycle. If your final payment is declined, blocked, or otherwise fails, your membership account will be placed in a suspended status. During this time, any remaining floats or service credits will be temporarily unavailable. To regain access to your credits, your outstanding balance must be resolved. If payment is not received within 180 days of cancellation, your unused credits will be permanently canceled and no longer eligible for use or reinstatement.

Float Expiration Policy *

(I AGREE) After cancellation, floats remain valid for 180 days, provided your account is in good standing. Credits are non-transferable and may not be used while your account is suspended due to unresolved payments.

BENEFITS OF MEMBERSHIP *

(I AGREE) 1) Members may purchase additional 60-min float sessions at $59 for themselves or on a Gift Card - 2) Members may receive up to 10 minutes of free oxygen after each redeemed Float session. 3) Members may share floats on their account with 1, 2, or 3 authorized users, per their monthly plan. (Basic:1, Plus:2, Premium:3; Authorized users may only be changed once per billing cycle by contacting the local spa. 4) Members may participate in Float Programs to enhance the float experience. RELEASE AND INDEMNITY *

(I AGREE) I participate in the activity at my sole risk and responsibility. I release, indemnify and hold harmless True REST Franchising LLC, its servants and agents, from and against all and any actions or claims which may be made by me or on my behalf or by other parties for or in respect of or arising out of any injury, loss, damage or death caused to me or my property whether by negligence, breach of contract or in any way whatsoever.
