Payment Terms *

(I AGREE) I understand that membership fees are recurring and will be charged monthly on the anniversary date of enrollment. The initial payment is due at sign-up, and all membership benefits will begin immediately. Subsequent charges will occur each month on the same calendar day. In months where the billing date does not exist (e.g., the 31st in shorter months), the charge will occur on the last calendar day of that month.

Notice to Cancel *

(I AGREE) that memberships may be canceled at any time with 30 Day written notice submitted to Glendale@TRUEREST.COM. All membership benefits will terminate immediately upon cancellation. If final payment is not received or cancellation procedures are not followed, the membership will be considered in default and all services and access will be revoked immediately. (I AGREE) Promotional Memberships require a 90-day minimum term and are subject to a 30-day written cancellation notice. (I AGREE) Membership cancellations take effect at the end of your current billing cycle. If your final payment is declined, blocked, or otherwise fails, your membership account will be placed in a suspended status. During this time, any remaining service credits will be temporarily unavailable. To regain access to your credits, your outstanding balance must be resolved. If payment is not received within 60 days of cancellation, your unused credits will be permanently canceled and no longer eligible for use or reinstatement. (I AGREE) After cancellation, services remain valid for 30 days (Limited Member Services for 60 Days), provided your account is in good standing. Credits are non-transferable and may not be used while your account is suspended due to unresolved payments.

Membership Benefits *

(I AGREE) Members may purchase additional float sessions for $59 and additional Vitality Suite sessions for $45, either for personal use or as a gift. Each appointment includes up to 10 minutes of complimentary oxygen therapy. (I AGREE) LIMITLESS MEMBERSHIP Members who purchase a Limitless Membership may use up to one service per day. Limitless Membership holders may float during any day or time AVAILABLE on our schedule. For all Limitless membership levels, memberships may not be shared. Memberships will only be honored at the spa in which they were purchased I AGREE) LIMITED MEMBERSHIP (GLENDALE ONLY) Members who purchase the Limited membership receive 1 60-minute float per month. Floats auto expire in 60 days. Limited membership level, may not be shared. Limited Memberships will only be honored in the Glendale location. Limited Membership holders do not receive any additional membership benefits. Limited memberships are a promotional membership and as such, they are subject to a 90-day membership period prior to cancelling

Release and Indemnity *

(I AGREE) I voluntarily participate in all activities and services offered by True REST at my own risk. I hereby release, indemnify, and hold harmless True REST Franchising LLC, its affiliates, officers, employees, and agents from any and all claims, liabilities, damages, or losses arising out of or related to my participation, including but not limited to injury, illness, or death.
